Chapter 633 Experts from the external hospital come to support

"Comrade, comrade, how are you doing?"

Chen Qi anxiously shouted several times to the three seriously wounded men, but received no answer. Two of them were groaning in pain, and the other seriously wounded man had been shouting for a long time but had no response. He probably fainted from pain.

"Uncle Guo, it is estimated that these three people have fallen into shock, or are in the early stages of shock."

In addition to the cause of the burn itself, continuous flushing of patients with large-scale deep burns with cold water will not only cause a variety of infections, but also cause a sharp drop in body temperature and blood pressure, leading to shock.

Shock is an important cause of early death in burn patients.

Guo Yuanhang also had a serious expression at this time: "There is no need to send him to the emergency room. He can be sent directly to the sterile ward of the burn department for debridement surgery."

Fortunately, these workers were all burned by molten steel and their respiratory tract damage was not serious. Otherwise, a tracheotomy would have been required on site.

Everyone took action. Even many doctors and nurses passing by saw the situation at the emergency scene and took the initiative to stay and help. Especially the three seriously injured people were sent to the burn department as quickly as possible.

Zhu Huoyan and the burn doctor are already ready and waiting.

When Lao Zhu saw the condition of the wounded, he felt secretly bitter in his heart.

We have just taken over Huang Tan's two patients with large-scale burns all over their bodies. We haven't yet figured out the experience. My dear, in the blink of an eye, three more seriously injured patients have arrived. It's very stressful.

And it looks like the burnt area exceeds 80%. Whether he can survive is a question.

It would be unlucky to have someone die just after opening, and it would also affect the reputation of the Burn Surgery Department of the People's Hospital.

But Zhu Huoyan is not a person who gives up easily. Now that the burn department has been opened, he will definitely fight for his life.

As the chief director of surgery, Zhu Huoyan immediately stepped into the role of a doctor and gave orders to his two "superiors":

"Xiao Chen, please be gentle on this wounded person. You are responsible for debridement. Guo Shuji, we will debridement the two most serious burn patients one by one."

Even if it's light, it won't make any difference. It's the difference between a 90% burn and an 80% burn.

Chen Qi and Lao Guo both nodded, and then took their respective wounded people into separate wards to prepare for first aid.

Then the first problem arose, because the worker suffered burns all over his body, and his skin was falling off in pieces. Much of his body was covered in coke, and there was no way to monitor his vital signs.

The problem is that this is an important indicator for treatment evaluation. Without this data, doctors cannot make further treatment decisions.

Chen Qi heard that the ward next door was in chaos, and several nurses were chirping:

"Oh, I can't even put on the cuff of this blood pressure monitor, there's no way to fix it!"

"This patient's outer lips are damaged, so I can't take his temperature. His anus is cut, so I have to take his rectal temperature!"

"The skin is so rotten that I can't measure my heartbeat and pulse rate. What should I do?"

After a while, Comrade Lao Guo's scolding came:

"Shout, what are you shouting for? Now that the master worker is in danger, why do you talk so much nonsense about asking you to test your vital signs? You are so precious, so disgusted with being dirty and so afraid of being sick, why don't you just go home and stay there, what kind of nurse do you want to do?

doctor?"

It seems that Lao Guo was really pushed into a panic. On the contrary, Zhu Huoyan seldom cursed others and was relatively cold-blooded.

Compared to the one next door, the rescue was going on in an orderly manner in the ward where Chen Qi was located.

Working with Chen Qi is Yang Xiuxiu, the deputy head nurse. She and Chen Li learned skills from Dean Chen in Africa. They are relatively experienced in burn rescue and will not panic when encountering problems.

Chen Qi curled his lips when he heard the scolding next door, thinking that the quality and clinical level of some medical staff really needed to be improved. This is also the huge difference between the Vietnam-China People's Hospital and top hospitals. There is a big gap in the quality of medical staff.

This people's hospital needs to be rated as a tertiary hospital. With the quality of its staff, it seems that it is really not good enough.

"Xiuxiu, later you will instruct them on how to rescue and nurse burn patients. If it really doesn't work, you make a list and I will transfer them. Poor skills are understandable, but having a bad attitude is unacceptable."

As the dean, Chen Qi gave her confidants a "Sword of Shang Fang", which helped her establish prestige in the department.

"Okay, Director Chen, I will open the intravenous channel for this patient first!"

The first step in resuscitation is to open the intravenous channel, that is, where to insert the needle?

Patients with extensive burns and burns all over the body have little good skin left, so traditional veins on the back of the hand or arms are simply not feasible.

And because of the need for rescue, when rescuing seriously injured patients, one channel is often not enough, and two or three channels are very common. Therefore, how to choose the injection site will test the nurse's basic skills.

Chen Qi glanced at Yang Xiuxiu. The little girl was wearing a white nurse's cap and a mask. Only her big eyes were exposed. She ignored the strong odor on the patient's body and kept her face close to the skin while looking for a suitable injection point.

Patients with severe burns usually choose unconventional operations on the radial artery or femoral artery.

At this time, you can't see the blood vessels. You can only touch them with your hands to carefully feel the location and direction of the arteries. Finally, you can insert the acupuncture based on your feeling. Ordinary nurses can't do it at all.

If it is more difficult, it will have to be completed under the guidance of B-ultrasound, which is very technical.

Chen Qi previously taught Yang Xiuxiu and Chen Li these arterial puncture and catheterization techniques in Africa. Now he just wants to see how Yang Xiuxiu performs?

An unmarried girl became the deputy head nurse. If she wanted to convince the public, she had to show her true skills. Otherwise, she would be overthrown by other nurses sooner or later if she couldn't control the situation.

You must know that the palace fights between women are very powerful, and it is not just Chen Qi who is the "financier father".

Yang Xiuxiu lived up to her expectations and opened two channels in three clicks. At the same time, she implanted a hollow needle in the artery in her wrist and connected it to the monitor.

A three-channel tube was also implanted in the large vein of the right neck. One channel monitors the patient's intravascular pressure, and the remaining two channels are used for infusion and medication injection during the operation.

This chapter is not over yet, please click on the next page to continue reading! The amount of fluid replenishment needs to be strictly evaluated, and it is very critical to rescue patients with burn shock.

Fluid replenishment is not something you can just replenish as much as you want. It needs to be calculated based on the human body's needs. So how do you know if you have over-replenished? At this time, you need to monitor the central venous pressure (cvp).

How to monitor the data? It is also very simple.

For example, if the central venous pressure and blood pressure are both reduced, it generally indicates that the blood volume is seriously insufficient, and adequate fluid replacement treatment should be given.

When central venous pressure is low and blood pressure is normal, it indicates insufficient blood volume, and appropriate fluid replacement therapy should be performed based on central venous pressure.

When central venous pressure is high and blood pressure is low, it indicates cardiac insufficiency, so be careful to prevent heart failure.

When the central venous pressure is high and the blood pressure is normal, it means that the blood vessel volume is excessively contracted, and vasodilation treatment should be performed.

See, is it very simple? Clear at a glance? Some people think that they can also be doctors.

So when technology changes medicine, who is more important, machines or people? Both are important.

To determine whether the fluid replenishment is in place, later generations of doctors only need to look at the central venous pressure, pulmonary artery wedge pressure, central venous blood oxygen saturation and other indicators on the display screen, and they can adjust the fluid replenishment volume and drip rate in time.

But what if we don’t have these monitoring instruments? For example, Huaguo Hospital in the 1980s.

Then we can only look at the patient's urine output, blood pressure, heartbeat, pulse, and mental state.

For example, is he restless or quiet? Is he obviously thirsty?

The above indicators can indeed reflect dehydration and whether fluid replenishment is adequate.

But they can only be used as a reference, and you cannot accurately monitor the body's fluid rehydration needs.

Moreover, the above judgments are often very subjective. For example, if a patient is irritable, is it because the fluid intake is insufficient, the blood volume is insufficient, or is the irritability caused by pain? It is too difficult for you to judge.

Often by the time you discover an abnormality, you have already overfilled it, or simply not done enough, and then the patient dies.

In the 1980s, it was not even possible to dynamically observe the changes in blood lactate of patients. In Chen Qi's view, all treatments were relatively crude, but this was the national situation, and Chen Qi had nothing to do.

The only thing he could do was to bring in 10 sets of the most advanced monitoring equipment from abroad. Who said that the Vietnam-China People's Hospital now has at least 10 million US dollars in its account? It is a standard big dog.

Therefore, the equipment of the Burn Department of Vietnam-China People's Hospital is definitely leading in the country and first-class in the world, but outsiders don't know it yet.

I'm just afraid that if it gets out, what will happen if all the major hospitals in the country come to "borrow" it?

Will you lend it out at that time? If you lend it out, you will be afraid that the meat bun will beat the dog and never come back. If you have money, you must not be robbed.

Now that he has money, Dean Chen Qi has ambitions. Either he will not build it, or he will build a world-class burn department. Coupled with his experience from later generations, it is hard to believe that it will not work out.

Seeing that Yang Xiuxiu successfully completed so many tasks that should have been completed by anesthesiologists, Chen Qi was extremely satisfied:

"Xiuxiu, hurry up and help Guo Shuji and Director Zhu. If you don't go, the old man's scolding will get louder and louder."

Yang Xiuxiu didn't have time to wipe the sweat from her forehead, nodded, picked up the toolbox and ran out quickly.

Chen Qi looked at Zhang Xing at this time: "Xiao Zhang, come on, we have to do arithmetic problems again. Let's calculate the patient's fluid rehydration amount first..."

While Chen Qi and the others were still seizing the time to rescue him, burn department experts from the provincial capital and Shanghai City were all on the way.

Xihu City is only 60 kilometers away from Central Vietnam and can be reached in one hour.

Shanghai is not far away either, 60 kilometers away, so it won't take long to get there.

Such a major liability accident occurred in the Yuezhong Steel Plant this time, and Director Qi also went all out, truly trying to save all the workers at all costs, and not a single person should die.

Although Yuezhong Chen Qi has proven his ability in the field of gastroenterology, the patient in front of him is a burn patient. Director Qi does not believe that Chen Qi can complete the rescue work well.

If the Yue Traditional Chinese Medicine Hospital makes a mistake and a few workers die, the fun will be great, and they will not want to lose the hat of the people.

Therefore, for the lives of workers and for our own interests, calling in foreign aid is the best way.

When the time comes, a red envelope of 1,000,000 yuan will be given, which is enough to send the experts away.

What does this amount of money mean to the Yue Steel Factory that has 30,000 people? The Yue Steel Factory sometimes entertains people with meals and drinks, and a meal can cost more than one thousand or two thousand.

Shanghai Dongshan Hospital.

As one of the best hospitals in Shanghai and Shanghai, Dongshan Hospital has the top burn department in the country.

When the hospital received a rescue call from Vietnam and China, Dr. Deng Changle, as the deputy director of the burn department, was designated by the hospital as the leader of the rescue team and was required to set off immediately.

Deng Changle has a younger brother named Deng Changxi.

Who is Deng Changxi? He is the director of hand surgery at UBS Hospital in Shanghai. He had fought with Chen Qi in the ring before, and the two sides had a very unpleasant quarrel.

Of course, in the aftermath of the ring incident, Wei Cheng of Sekisuitan Hospital, who was the mastermind behind the incident, immediately stepped down and was removed from his position as president of the Society of Hand Surgery and director of the hospital's hand surgery department.

Also affected were two major gangsters.

One is Wu Jianlin from the First Hospital of Hainan Medical University, and the other is Deng Changxi from UBS Hospital. Both of them were also removed from their positions as department directors and demoted to ordinary doctors.
